When it comes to delivering an IV drip, it’s more than just boosting their bodies— it’s about caring for the whole human. Whilst stepping into a clinic might be a typical work day for us, for a new patient, especially one receiving IV or IM treatments– it often feels very daunting. It can come from anywhere from fear of needles, sensory needs, to just general medical anxiety. How we support patients in those moments of elevation can make a big difference.
One simple, powerful way to help patients feel calm and cared for is through grounding techniques. These are basic exercises engineered with the intention of reconnecting the recipient with the present moment and helping them feel more in control of their experience. They also often follow a theme of breaking an overload of mental input into small, digestible pieces. Below are a few tried-and-true clinic-friendly grounding techniques that you and your team can incorporate into your practice.
1. The 5-4-3-2-1 Method
This sensory-based technique is a simple way of shifting the focus away from fear or physical sensations associated with an elevated nervous system. Ask them to name:
5 things they can see — “Can you name five things you see in the room?”
4 things they can touch — “Notice four textures around you — your chair, your clothes…”
3 things they can hear — “What three sounds do you hear right now?”
2 things they can smell — “Any scents around? Perhaps the tea, the sanitiser…”
1 thing they can taste — “Take a sip of water or tea and notice the taste.”
Remember that your demeanour and vocal tone are important too ! Be gentle, calm, and encouraging. Rapid, loud, apathetic or impatient speech patterns will simply encourage further stress.
In a similar vein, don’t assume that just because physical touch may comfort you that it will comfort a stranger. Always ask ! Some people may appreciate their hand being held, but for others that may be their worst nightmare. In contexts where physical touch is necessary, remember to announce where and why :).
2. Breathing Exercises
Anxiety often shows up as shallow or rapid breathing. You can guide the patient through a simple breathing rhythm while you prep their drip:
Breathe in for 4 counts, hold for 4, breathe out for 6.
Repeat 3–5 times.
Why it helps: Slow, rhythmic breathing sends a calming signal to the nervous system. It also offers the mind something else to focus on.
3. Tactile Grounding Tools
Consider having a few discreet “fidgets” available at your clinic — soft stress balls, a cool stone, textured fabric squares, or a weighted lap blanket. Let new patients know that they’re available and within reach if they wish. This removes any shame or stigma associated with fidgeting and can be a wonderful first step to show your patient that they’re in a safe place 🙂
Why it helps: Tactile sensations can help regulate feelings of unease and provide texture experiences to focus on.
4. Give Permission to Pause
Sometimes, simply letting patients know that it’s okay to take things slow can help them relax. Before you insert the needle, say something like:
“There’s no rush — let me know when you’re ready, and we’ll go step by step.”
Why it helps: Empowering patients with choice makes them feel more in control in an uncertain environment.
5. Create a Calming Environment
When patients feel more at ease with their surroundings, they will feel calmer to brace for the otherwise daunting moment. Even the clinic atmosphere itself can be grounding with simple changes:
Play soft, neutral music.
Offer sunglasses or even a sleeping mask to help people with sensitivities feel more comfortable in bright lighting.
Offer warm tea or a blanket.
